Services Include:
- Management of symptoms and quality of life issues
- Help communicating clearly with the family, doctor and care team
- Help to clarify healthcare needs and goals as they change
- Decision making help: practical and emotional support; counseling
- Access to community resources
Providence Connections
A free non-medical in-home service (supported by Providence Community
Foundation). Services available to those with life-limiting illness and
less than one year life expectancy.

Palliative Social Worker: Marty Hogan, LCSW
Services Include:
- Access to and coordination of community services and resources
- Assistance with advance care planning
- Help navigating the healthcare system
- Help with chores, meals and errands
- Transportation to medical appointments
- Friendship and companionship
- Respite for family members

Southern Oregon Friends of Hospice gifts include:
- Providing educational material to patients and families,
as well as emergency assistance for heating, electricity and transportation
costs.
- Funding a day trip as respite care for overwhelmed family
members of a dying patient.
- Supplying respite caregivers for a family exhausted from
24-hour care-giving.
- Reimbursing harpists providing music therapy, and massage
therapists providing therapeutic touch for hospice patients.
- Offering bereavement support groups and training for volunteer
grief counselors.
